The immigration medical examination, documented on USCIS Form I-693, is required for most applicants adjusting status to lawful permanent resident (a “green card”). It must be performed by a USCIS-designated Civil Surgeon, who confirms you are free of certain communicable diseases and meet vaccination requirements.
Our immigration medical exam is $200 self-pay. Required vaccinations and certain lab tests may be additional depending on what you need. We’ll go over any costs with you before proceeding. Immigration exams are generally not billed to health insurance.

No. You may fill out your portion in advance, but do not sign it. USCIS requires that you sign the form in the presence of the Civil Surgeon at your appointment.
The Civil Surgeon completes, signs, and seals your Form I-693 in the official USCIS envelope. Do not open the envelope — submit it to USCIS with your application exactly as sealed. USCIS will reject an opened form.
A properly completed Form I-693 is generally valid indefinitely as long as it was signed by the Civil Surgeon no more than 60 days before you file your adjustment-of-status application, though you should confirm current USCIS policy. We’ll help you time your exam correctly.
